When you sign up to any of our programs, you will receive the Player and Parent agreement alongside a liability waiver that must be signed in order to complete the onboarding process

The following documents are just templates.
SOLO FUTBOL - PLAYER CONTRACT AGREEMENT
This agreement is made between SOLO FUTBOL ("the Program") and the undersigned participant ("Player") or their parent/guardian (if under 18). By signing this contract, the Player and/or Parent/Guardian agree to the following:
A. Introduction
Please review this agreement carefully to understand the expectations of SOLO FUTBOL. Should you have any concerns, contact us at contact@solo-futbol.com. By enrolling in our program, you agree to these terms. If you disagree with our terms, please do not proceed with enrollment.
B. Agreement Term
Start Date: The date of registration.
End Date: Continues until cancellation is requested.
Cancellation: Requires 30-day written notice to cancellations@solo-futbol.com before the next billing cycle. Early cancellation incurs a fee equivalent to one month's payment. Automatic renewal applies without proper cancellation notice.
C. Behavior Expectations
Player Commitments:
Players must attend all scheduled sessions and arrive on time (this means 15 mins prior to the start of the session)
Players arriving after the initial whistle is blown will not be allowed to practice. The session will be counted as a cancellation within the 24 hour grace period.
Players must exhibit a positive, coachable attitude.
Players must show respect towards coaches, teammates, and facilities always.
Players must wear proper soccer attire and bring necessary equipment to every session. Any form of disrespect, aggression, or misconduct will not be tolerated and may result in removal from the program without a refund.
Any absence must be communicated at least 24 hours in advance. Repeated absences or tardiness may result in suspension from the program.
In the event that the SOLO FUTBOL needs to move/rescheduled a session for whatever reason (holiday, health reasons, weather, etc.), it's your responsibility to
make the session.
Parents Commitments:
Understand and accept the no-refund policy.
Ensure punctuality for your child's sessions.
Communicate via email for any inquiries.
Acknowledge weather, payment, refund, injury, and social media policies.
D. Policies
Repeated absences or tardiness may result in suspension from the program.
We have a zero-tolerance missed session policy. If you miss a session without notice, you will forfeit the session. We respectfully request at least 24 hours advance notice for all rescheduling/cancellations.
If you communicate with us more than 24 hours before the missed session, you will receive a makeup session by the end of your training agreement. If you communicate with us less than 24 hours before the session, you will forfeit the session and no makeup time will be available. Max 1 make up session per subscription period.
Weather Policy
If the field is too wet or if there is significant rain during the morning or evening of our scheduled session, the session may be rescheduled upon SOLO FUTBOL staff's decision. SOLO FUTBOL staff will check to ensure the field is safe before every session. If the field is playable, we will resume the session. Parents do not determine if the session is canceled. If a parent decides not to attend a session that has been deemed “playable”, then that session will count as a cancellation of less than 24 hours and will not be eligible for makeup.
In the occurrence of lightning during a session, we will end the session right away to seek shelter. Any lightning strike within a 10 mile radius will end the session (according to WeatherBug). If this occurs 15 minutes or more into the session then the session will count and will have to be abandoned. Safety is our number one priority. If it's within 15 minutes of the start time then we will reschedule the session.
Payment Policy
By signing up with SOLO FUTBOL, you automatically agree to our Terms & Conditions, you agree with the contract that you can commit to the entire training period. You have the option of paying, quarterly, semi-annual, or yearly with our automated system that charges your credit/debit card every 90/180/365 days. If your credit card fails, our system will prompt you to replace your card within a 24-hour period. If a new card is not registered within 24 hours, then all sessions will be stopped until payment is collected and the time slot may be lost.
For any upgrades on your subscription, please let us know 7 days before your current subscription expires so we are able to set up your account with the new one.
Refund Policy
Due to the demand for our programs, we do not offer refunds for ANY case for any program.
Once a player reserves a training spot, we hold the spot for the player for the specific program.
Cancellation Policy
Agreement renews automatically every 3, 6, or 12 months, so If you want to CANCEL your membership, please notify us 30 days BEFORE the end of our agreement. By doing so, we will cancel the account. If not, you are auto-renewed into the next training term. You can notify us in writing at (cancellation@solo-futbol.com) at least 30 days before our final session and we will turn off the membership.
Injury Policy
If injury occurs and a player is unable to participate in the training sessions, the recurring payments will continue until the last day of the Training Agreement. Once the player has healed, we will then continue our sessions until all sessions have been completed. We understand that injuries are a big part of the sport so we try to be flexible.
Social Media Release Policy
“I, the undersigned, do hereby grant permission to SOLO FUTBOL to post my and/ or my child’s story, photo, videos, hereinafter referred to as “Materials,” taken by SOLO FUTBOL during sessions or that I submit to and for the SOLO FUTBOL website and social media accounts.
I hereby release you, your representative, employees, managers, members, officers, parent companies, subsidiaries, and directors, from all claims and demands arising out of or in connection with any use of said “Materials”, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights”.
Health and Wellness Policy
“I have enrolled in the personalized health and fitness program offered through SOLO FUTBOL.
I recognize that the program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.
I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this exercise program. I acknowledge my enrollment and participation in SOLO FUTBOL training.
“I fully understand that I may injure myself as a result of my enrollment and participation in this program and I hereby Release and Forever Discharge SOLO FUTBOL and its agents, employees, representatives, affiliates, successors, or assigns, from any and all liability now or in the future for any conditions, injuries, sickness, losses, expenses or damages that I may obtain or incur. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other soreness that I may incur, including death.”
D. Agreement
This document confirms the understanding and agreement to the terms and conditions outlined above between SOLO FUTBOL and the undersigned. Failure to comply with these terms may result in termination of services without refund.
By signing below, the Player and/or Parent/Guardian acknowledge that they have read, understood, and agreed to the terms outlined in this agreement.

SOLO FUTBOL – Liability Waiver and Release of Claims
1. Acknowledgment of Risks
I, [Player’s Full Name], or the undersigned parent/legal guardian if the participant is under 18, acknowledge that participation in soccer training activities with SOLO FUTBOL involves inherent risks. These risks include, but are not limited to:
Collisions with other players, equipment, or the ground
Sprains, strains, fractures, concussions, or other physical injuries
Weather-related conditions, dehydration, and fatigue
Unforeseen accidents or medical emergencies
I understand that while SOLO FUTBOL and its coaches take reasonable precautions, injuries may still occur. I voluntarily assume all risks associated with participation in training sessions.
2. Release of Liability
In consideration of being allowed to participate in training programs, I hereby release, waive, and discharge SOLO FUTBOL, its owners, coaches, trainers, employees, agents, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of:
Personal injury, illness, or death resulting from participation
Property damage or loss sustained during training
Any medical expenses incurred due to injuries sustained during training
This waiver applies whether injuries are caused by negligence or otherwise, to the fullest extent permitted by law.
3. Medical Treatment Authorization
In the event of a medical emergency, I authorize SOLO FUTBOL coaches or staff to administer basic first aid and seek emergency medical treatment if necessary. I understand that I am responsible for any medical costs incurred.
4. Insurance Responsibility
I acknowledge that SOLO FUTBOL does not provide medical or accident insurance for participants. I confirm that I have adequate insurance coverage or accept full responsibility for any medical expenses.
5. Indemnification Clause
I agree to indemnify and hold harmless SOLO FUTBOL, its owners, coaches, trainers, employees, and affiliates from any claims, lawsuits, or liabilities brought by myself, my family, heirs, or legal representatives related to participation in training activities.
6. Parental/Guardian Consent (For Participants Under 18)
If the participant is under 18, I, the undersigned parent or legal guardian, consent to their participation and agree to all terms outlined in this waiver.
7. Agreement and Signature
I have read and fully understand this waiver. I acknowledge that I am voluntarily signing it and that it shall be binding upon me, my heirs, executors, and legal representatives.
